My name is Sin Yi Sara Ho. I reside in Malta and am the biological mother of Yvre Merlin (born April 1, 2011). Her father, Maximillian Merlin, currently has full legal and physical custody under an existing Florida court order. He lives in Florida and is her primary caregiver.
We have now mutually agreed to modify this arrangement: • We both support shared legal custody • We agree that Yvre will relocate to Malta to reside primarily with me during the school year • Yvre has been accepted to two schools in Malta (St Andrea and St Martin’s), and she will choose where to enroll • Her father will maintain parenting time during holidays and breaks, including summer and Christmas, and may visit her in Malta
We have drafted the following documents: 1. Supplemental Petition to Modify Parenting Plan and Time-Sharing 2. Mutual Custody Modification Agreement and Relocation Consent 3. Modified Parenting Plan 4. Parental Consent to Relocation 5. Filing checklist and procedural outline
