Branded Diagnostic Questionnaire with Scoring plus Bar & Radar (Spider) Charts in WordPress Overview I am looking for an experienced WordPress developer to build a polished, on-brand diagnostic tool for my consultancy website. The project includes: • A 25-question diagnostic questionnaire • Automatic scoring mapped to 10 diagnostic themes • Visual output as both a bar chart and a radar (spider) chart • Seamless visual integration so it looks and feels native to the website I do not need data pushed into Excel as long as I can see each individual response and its scores clearly in the WordPress admin (and optionally on a results page). This is primarily a configuration project using established WordPress form and chart plugins.
Branding requirement (critical) The diagnostic must: • Look like an integral part of the existing website • Match the site’s typography, colours, spacing and general look and feel • Use my brand palette consistently • Sit naturally within a standard page template • Present charts that look cohesive with the rest of the site This should not feel like a generic plugin form dropped into the page. It should present as a purpose-built NorthCo diagnostic. Deliverables 1. Online Diagnostic Form • 25 questions, each presented as a dropdown • Each dropdown option corresponds to a predefined numeric score (I will provide the full mapping) • Questions are grouped into logical sections • Clean and fully on-brand layout aligned with the website’s visual style • Fully responsive on desktop, tablet, and mobile 2. Scoring Logic • Each dropdown option corresponds to a specific score value • Each question belongs to one of 10 diagnostic themes • Theme totals (or averages) must be calculated automatically on submission • Theme scores must be clearly visible for each individual response • These theme scores must feed both chart types • No custom coding required unless absolutely necessary (plugins can handle this) 3. Visual outputs For each completed diagnostic, the system should generate: • A bar chart showing the 10 theme scores • A radar (spider) chart showing the same 10 theme scores Charts should: • Use the website’s branding and be visually tidy • Sit neatly within the page layout • Be visible either: o On a respondent-facing results page, and/or o In an admin-only view for my internal use I am happy to confirm the exact flow once you outline the options. The priority is that I can easily view the charts per respondent. 4. Response management • All submissions stored securely in WordPress • Ability to view each respondent’s answers and their theme scores on a per-submission basis • If possible, the ability to print or save a clean results view (including charts) for my own records or to share with the client as a PDF or similar I do not require an automated Excel export, as long as the individual responses and scores are easy to review and refer back to. 5. Embedding and integration • Embed the diagnostic on a specified WordPress page • Ensure forms and charts inherit the site’s styling and do not look out of place • Provide clear instructions or short documentation so I know how to: o Edit text in the form later o Change basic labels o Access responses and charts
Preferred plugins and tools • Gravity Forms plus a suitable charts add on (such as GravityCharts or GravityView with charts), or • Formidable Forms (with built in calculations and charts), or • Fluent Forms combined with a charting solution that supports radar charts Your approach must support both bar and radar (spider) chart visualisations. Please state in your proposal: • Which plugins you intend to use • How you will produce both bar and radar charts • Whether you have examples of similar work If premium plugins are required, please confirm whether you can use an existing developer licence or if you will need me to purchase licences.
What I will provide • The 25 diagnostic questions • The 10 diagnostic themes • Mapping of which questions belong to which theme • Brand colours and any styling guidelines • The page where the diagnostic should live • Confirmation on whether charts should be visible to respondents, internal only, or both
Success criteria The project will be considered successful if: • The diagnostic looks and feels like a natural part of the website • Users find the form straightforward and professional to complete • Theme scores are calculated correctly and stored per submission • Bar and radar charts are generated reliably for each response • I can easily review individual responses and scores through the WordPress admin (and results page, if used) • The solution is stable, maintainable and does not rely on fragile custom code
Skills required • Strong WordPress experience • Comfortable working with form and chart plugins • Experience with calculated fields and scoring • Ability to integrate the design cleanly with an existing site • Good attention to visual detail and UX • Clear and reliable communication
